Register for Disability Resource Services at Eastern Arizona College

Disability Resource Services provides a range of accommodations and support to ensure equal access for students with disabilities.

The following steps outline how to request services, submit documentation, and work with the Coordinator to develop an accommodation plan that supports your academic success.

  1. Contact EA’s Disability/504/ADA Coordinator to schedule an in-person, phone, or Zoom appointment:

    Note: Students must self-identify (request services) at the college level.

  2. Provide psychoeducational, psychological, psychiatric, or other appropriate diagnostic documentation that verifies the nature and extent of the disability (in person or via email).
  3. After documentation is reviewed, meet with the Coordinator to participate in the interactive process and develop an accommodation plan.
  4. Sign a Release of Information.

  5. Participate in a student interview. Be prepared to discuss:

    • How your disability substantially limits you in the educational environment
    • The barriers or challenges you are experiencing
    • Your history of accommodations
    • Accommodations that may help remove those barriers
  6. Complete and sign a Request for Disability Services for the specific semester.
  7. Disability Resource Services will contact your instructors and create an accommodation plan.
  8. Sign the accommodation plan for the semester. The plan will be shared with you and your instructors.

Important: Each semester, you must request disability services.
