Disability Resource Services

EAC Disability Resource Services leads collaborations  with admitted and prospective students with disabilities, faculty, and staff to remove barriers through a universally designed environment and  provide accommodations for equal access in compliance with Section 504 of the Rehabilitation Act of 1973 and Americans with Disabilities Act (ADA) of 1990. We recognize disability as an aspect of diversity and promote a culture of inclusion.

What is the process to register for Disability Resource Services at Eastern Arizona College?

 

  1. Contact and meet with EAC’s Disability/504/ADA Coordinator, Candace Lines.  Please call 928-428-8425 to make an in-person, phone or Zoom appointment.  Students must self-identify (request services) at the college level.
  2. Provide EAC psycho-educational, psychological, psychiatric, or other appropriate diagnostic evaluation that documents the nature and extent of the disability in person or by email at disabilityresourceservices@eac.edu.
  3. Once the necessary documentation has been provided and reviewed, meet with EAC’s Disability/504/ADA Coordinator to participate in the interactive process to develop an accommodation plan.
  4. Sign a Release of Information.
  5. Participate in a student interview.  Be prepared to answer the following questions:
    1. How does your disability substantially limit you within the educational environment?
    2. What are the difficulties or barriers you feel you are facing?
    3. What is the history of your past accommodations?
    4. What type of accommodations do you feel may remove those barriers?
  6. Complete and sign a Request for Disability Services for the specific semester.
  7. Disability Resource Services will then contact your instructors and create an Accommodation Plan.
  8. Sign an Accommodation Plan for the specific semester.  The Accommodation Plan will then be sent to you as well as the instructors you specified.  Each semester, you must request disability services.
